To request a repeat prescription please upload your order form below. Order forms can be obtained from reception.

To ensure a safe and efficient service we can only accept re-order forms issued from us or hospital prescriptions. 

The form can be used to request your regular medication only. If you need a prescription for a new medication or something not prescribed in a long time, you will need a consultation with a doctor.

Patients are asked to attend their doctor for review on a regular basis.

Please allow 48 hours (not including weekends) for request to be processed.

For private patients (patients with no medical or doctor visit card) you can pay the fee of €15 securely by following the link on this website.
