To request a repeat prescription please upload your order form below. Order forms can be obtained from reception.

To ensure a safe and efficient service we can only accept re-order forms issued from us or hospital prescriptions. We CANNOT accept any other means of communication here. You WILL NOT receive a reply. If you wish to convey a message to the doctor you must contact the Surgery directly.

The form can be used to request your regular medication only. If you need a prescription for a new medication or something not prescribed in a long time, you will need a consultation with a doctor.

Patients are asked to attend their doctor for review on a regular basis.

Your prescription will be emailed to the pharmacy you got your last prescription. If you wish to change Pharmacy please contact us. 

Please allow 48 hours (not including weekends) for request to be processed.

For private patients (patients with no medical or doctor visit card) please note a fee of €15 applies to all prescriptions.
